SSS Form E-1: This is the SSS member registration form, which is used to register as a member of the SSS.

SSS Form ML-1: This is the maternity notification form, which is used by female members to notify SSS of their pregnancy and intended date of delivery.
SSS Form DDR-1: This is the disability notification form, which is used to notify SSS of a member's disability and claim for disability benefits.
SSS Form RS-1: This form is used to apply for a Social Security ID card, which serves as a valid ID for SSS transactions.
SSS Form R-5: This form is used to apply for a salary loan, which is a short-term loan granted to members who are currently employed.
SSS Form UMID-SSS: This form is used to apply for a Unified Multi-Purpose ID (UMID) card, which serves as a valid ID for SSS, PhilHealth, Pag-IBIG, and other government agencies.

If you need to change any information in your record, you must report it immediately to any SSS office through the Member’s Data Amendment Form (SS Form E-4) and submit with the original/certified true copy and photocopy of the following:
Marriage certificate for change of civil status;
Birth or baptismal certificate of children for new additional children;
Birth or baptismal certificate or passport for correction of name and date of birth.
